You will need to print these forms, complete them and bring them with you to your first visit. New patient registration forms in Spanish are available in the office. Please let us know if you would like these forms mailed to you.

If you are an existing patient and have been asked to update your health information, please select the Health History Update Form below. You will need to print the form, complete it and bring it with you to your next appointment. Please remember to bring a list of medications you are taking.

Directions and Parking

We are located in historic downtown Gainesville. The office is located on the north east corner of California Street and Red River Street. Parking is available in front of the office on California Street, in the southbound direction on Red River Street and behind the office in our parking lot located off of Red River Street.
